However, e-cigarettes still pose health risks, even without nicotine.

Cig-a-likes may also contain low levels of cadmium, a toxic metal also found in cigarette smoke that can cause breathing problems and disease.11 More research is needed on the health consequences of repeated exposure to these chemicals.
The harmful effects of CS and their deleterious consequences are both well recognised and widely investigated. However, and based on the studies carried out so far, it seems that e-cigarette consumption is less toxic than tobacco smoking. "Since most of the health concerns about e-cigarettes have focused on nicotine, there is still much we do not know about e-cigarettes. The present study utilized a nationwide school survey which covers the majority of the respective Finnish student population and a validated measure for susceptibility, adapted to cover also e-cigarettes and snus. The limitations include self-reporting, lack of class-level data and a measure on school performance, and potential bias in parental education and smoking when reported by students. Further, no causal conclusions can be made from this cross-sectional data. Some adolescents experience first dependence symptoms early [2], which makes prevention of nicotine use essential.

E-Cigarettes, also called personal vaporizers or "vapes", present another way for smokers to ingest nicotine. E-Cigarettes have been marketed to young adults and adolescents through the use of candy and fruit flavors. They are also touted by some users as a "safer" alternative to smoking, and as a way to either quit smoking cigarettes, or to smoke in places where cigarette smoking is not allowed.
Vape flavors like mint and mango appeal to young people and mask the harshness of nicotine. The small, discreet size of vapes and e-cigarettes make them easy to conceal at school and home. The bottom line is e-cigarettes and vapes are unsafe for kids, teens and young adults. Nicotine is harmful to the developing brain and the use of e-cigarettes as a teen increases the likelihood of smoking cigarettes as an adult. The chemical additives and flavoring can also cause unwanted health effects.
Upon the selection of the educational track, students may already have normative perceptions of smoking in vocational schools, such as understanding of smoking as an integral and expected practice in these schools and professions [8]. These perceptions may increase S-SM and other nicotine product use, also among students with better academic performance. Student network characteristics and their associations with smoking vary between schools [28].
Bianco recalled an instance where her son was even subject to content in which a fishing YouTuber he watched was sponsored by a spitless tobacco brand. He notes that the most effective methods involve some sort of pharmaceutical, either nicotine replacement patches or pills, combined with a program that helps you deal with the problems of quitting smoking. (The American Lung Association recommends Freedom From Smoking.) Even then the average quit rate is just 25 percent, so even the best methods only have a one in four chance of succeeding. While saturated fat and alcohol still have their supporters, nobody is rushing to cigarettes’ defense. The new study included more than 1,500 adolescents in the United States, ages 13 to 17, who reported vaping in the previous 30 days, with many of them — about 76% — reporting that they vape within 30 minutes of waking up, a signal of nicotine dependence. Moreover, people who both vaped and smoked were four times more likely to go on to develop lung cancer compared to people who had only smoked, Bittoni's group found.
Nonetheless, adults who smoke who switch to using e-cigarettes expose themselves to potentially serious ongoing health risks. If they are unable to quit e-cigarettes on their own, they should seek help from a health care professional or quitline. Individuals who are not yet able to stop using e-cigarettes should be strongly discouraged from simultaneous, or "dual," use of any combustible tobacco products, including cigarettes. Continuing to smoke exposes the individual to enormous harms, irrespective of whether the individual is using e-cigarettes part of the time. All individuals should also be strongly counseled to not revert to smoking. Some look like a regular cigarette, but many resemble everyday products like pens, USB drives, highlighting markers, or colorful toy-like items.
To truly help tobacco users quit and to strengthen global tobacco control, governments need to scale up policies and interventions that we know work. Tried and tested interventions, such as brief advice from health professionals, national toll-free quit lines and mobile and digital cessation services are recommended. Where economically feasible, governments should also consider promoting nicotine replacement therapies and non-nicotine pharmacotherapies for cessation. E-cigarette emissions typically contain nicotine and other toxic substances that are harmful to both users and non-users who are exposed to the aerosols second-hand.

The inhaled vapor may contain nicotine (the addictive drug in tobacco), flavorings, and toxins—including ones that cause cancer. People who have already switched completely from smoking to e-cigarettes should not switch back to smoking (either solely or along with e-cigarettes), which could expose them to potentially devastating health effects. It's important to know the US Centers for Disease Control and Prevention (CDC) has stated that sometimes e-cigarette products are changed or modified and can have possibly harmful or illegal substances from unknown sources. Fortunately, there has been a substantial drop in cases since they peaked in August and September of 2020. What’s more, researchers have also identified vitamin E acetate, a chemical added to some THC-containing vaping products, as the main—but possibly not the only—cause of the illness. The outbreak of EVALI cases emerged against a backdrop of an ongoing vaping epidemic among youth.
However, many young users report they began using e-cigarettes because of the flavors and, according to CDC, do not realize they are inhaling nicotine. E-cigarettes are relatively new, and their long-term effects are not yet known. Although they may potentially be less harmful substitutes for traditional cigarettes, risks of addiction, injury, and death from e-cigarettes are becoming more apparent. Wehrli noted that they observed these striking changes after the participants (all of whom never smoked previously) used an e-cigarette a single time.
Read this paper by NCI’s Dr. Ned Sharpless and CRUK’s Dr. Iain Foulkes. Drug forums and e-liquid vendors were monitored by the researchers for e-liquids that "purportedly contained alternative pharmaceuticals." A clue in searching for e-liquids that contain illicit drugs is cost, the scientists said. Most nicotine e-liquids range from $5 to $10, while alternative drug e-liquids go for five to twenty times as much. In comparison to the general population, support for these measures varied greatly among people who currently used e‑cigarettes. The most supported measure was prohibiting the sale of e‑cigarettes to people under 18 years of age (67%), while the least supported measure was banning all additives in e‑cigarettes, to make them less attractive to young people (33%) (Figure 7).
We would argue that further studies with chronic administration of low doses of nicotine are required to clearly evaluate its impact on carcinogenicity. In this line, a study compared the acute impact of CS vs. e-cigarette vaping with equivalent nicotine content in healthy smokers and non-smokers. Both increased markers of oxidative stress and decreased NO bioavailability, flow-mediated dilation, and vitamin E levels showing no significant differences between tobacco and e-cigarette exposure (reviewed in [20]). Inasmuch, short-term e-cigarette use in healthy smokers resulted in marked impairment of endothelial function and an increase in arterial stiffness (reviewed in [20]). Similar effects on endothelial dysfunction and arterial stiffness were found in animals when they were exposed to e-cigarette vapor either for several days or chronically (reviewed in [20]).
Manufacturers claim e-cigarettes are a safe alternative to smoking regular cigarettes. Cancer is definitely a concern, given that vaping introduces a host of chemicals into the lungs. But vaping products haven’t been around long enough for us to learn whether or not they cause cancer. In 2018, 3.6 million U.S. high school and middle school students smoked an e-cigarette within 30 days of polling, making it the most common tobacco product used among this group.
At that point, levels of formaldehyde exceeded limits set by the California Office of Environmental Health Hazard Assessment (OEHHA), though other compounds didn’t come close. About a quarter of middle and high school students surveyed in 2017 said they had been around someone vaping at least once in the past 30 days. In 2021, only about 11 percent of high school students and 3 percent of middle school students surveyed said they had tried e-cigarettes. Results from a clinical trial published by JAMA Internal Medicine found that young adults ages who used This is Quitting from Truth had nearly 40% higher odds of quitting compared to a control group.
Similar results emerged when they excluded individual states with statewide policies from the analysis, showing that no one state was driving this effect. Bans on all vapes, including both flavored and unflavored, also resulted in more cigarette sales. The authors used rigorous statistical tools to estimate long-term effects of e-cigarette flavor restrictions on electronic and conventional cigarette sales in jurisdictions across 16 states. They also examined how these effects varied between brands disproportionately used by underage youth versus adults. However, within the adult demographic, 12.7 percent of 18- to 24-year-old adults and 21.3 percent of males, ages 18-24, are "current users" of e-cigarettes. The e-cigarette device (Vype ePod1.0, Nicoventures Trading Ltd., Blackburn, UK) consists of a metallic outer device case, a printed circuit board to control the device, a lithium-ion rechargeable battery (350 mAh) and an e-cigarette cartridge (Fig. 1). The voltage ranges from 2.2 to 3.1 V and is not adjustable by the user. The device has dimensions (h × w × d) of 104.2 × 19.1 × 10.5 mm and a power output of 6.5 ± 0.5 W. The cartridges or pods consist of a plastic case holding the ceramic wick material and a flat metal heating element (NiCr, 0.8 –1.4-Ω resistance). Each pod is pre-filled with Vype e-liquid (1.9 mL) and is magnetically attached to the device.
At least one of the three chemicals was detected in 47 of the 51 flavors tested. Diacetyl was detected above the laboratory limit of detection in 39 of the flavors tested. Federal, state, and/or local governments need to set clear standards on environmentally responsible e-cigarette waste disposal and hold the industry accountable for adhering to them. Researchers in a 2018 study found that cinnamaldehyde (found in cinnamon), o-vanillin (found in vanilla), and pentanedione (found in honey) all had toxic effects on cells. Cumberland County piloted CATCH My Breath in 2022, and had teachers in the restorative justice classrooms of Howard Learning Academy Middle School and Pine Forest High School run the program. The pilot was enough of a success that the county is now training one person in every middle and high school in the district to implement the program on a needs-basis in restorative justice classrooms this school year. There were also participants in a "waitlist" group, who received neither text messages through the This is Quitting program nor the assessment-only follow-up messages. This ban builds upon legislation signed in July by Governor Cuomo that immediately banned the use of e-cigarettes on all public and private school grounds in New York State. Today, nicotine poisoning has become a growing concern because of new nicotine products on the market, particularly electronic cigarettes (e-cigarettes) and pure liquid nicotine. The concentration of nicotine in liquid products is higher than most other tobacco products.

This is known as "dual use." The dual use of e-cigarettes and tobacco cigarettes can lead to significant health risks because smoking any amount of regular cigarettes is very harmful. People should not use both products at the same time and are strongly encouraged to completely stop using all tobacco products. Beyond the three main ingredients, some researchers worry about by-products from heating electronic cigarettes and the solution inside them.
Conducted from October 2021 to October 2023, the study involved 1,503 adolescents ages from across the United States who reported past 30-day e-cigarette use and were interested in quitting. Compared to a control group, participants who received the interactive text message program were 35% more likely to report not using nicotine at the 7-month study endpoint. Quit rates were 37.8% in the intervention groups compared to 28% in the control group. In North Macedonia, Italy, Malta, Austria, Luxembourg, and Belgium, the age for legal vending is 16. Since January 1, 2007, all cigarette machines in public places in Germany must attempt to verify a customer's age by requiring the insertion of a debit card.
While e-cigarette use prevalence has declined significantly since the heyday of JUUL — 10% of high school students reported using e-cigarettes in 2023 — e-cigarette use among young people remains a concern. Thousands of flavored, high-nicotine, and relatively cheap e-cigarette products remain on the market — many of them illegally — driving youth use and nicotine addiction. Equally concerning, nearly half of young people who have ever tried e-cigarettes continue to use them, and many do so daily. As encouraging as the data was a few years ago, it’s starting to look like that’s not the case. The FDA is yet to approve them as a smoking cessation aid and a recent CDC study found that most adult e-cigarette users — 58.8 percent of them — don't stop smoking cigarettes and instead wind up using both products. The few scientists actively trying to fill the gap in the research literature are running into obstacles.
Vapor products have many names, including electronic nicotine delivery systems (ENDS), electronic cigarettes (e-cigarettes), JUULs or vape pens. All adults who smoke conventional cigarettes or other combustible (burned) tobacco products should be advised to quit smoking at the earliest opportunity, recognizing that quitting is hard and often takes repeated, dedicated efforts. Individuals can also seek cessation support by calling QUIT-NOW or ACS-2345.
The FDA does not currently require testing of all the substances in e-cigarettes to ensure they are safe. It's also hard to know exactly what chemicals are in an e-cigarette because most products do not list all of the harmful or potentially harmful substances contained in them. E-cigarettes do not contain tobacco, but many of them contain nicotine, which comes from tobacco. Because of this, the Food and Drug Administration (FDA) classifies them as "tobacco products." For adults who smoke, switching completely from cigarettes to e-cigarettes may reduce exposure to many harmful chemicals present in cigarettes. However, it is important that they switch completely from cigarettes to e-cigarettes to get the full health benefit.
Electronic nicotine dispensing systems (ENDS), commonly known as electronic cigarettes or e-cigarettes, have been popularly considered a less harmful alternative to conventional cigarette smoking since they first appeared on the market more than a decade ago. Both the electronic devices and the different e-liquids are easily available in shops or online stores. The long-term health effects of electronic cigarettes (e-cigarettes) are not fully understood, but the science indicates they are not a safe alternative to smoking. Most are noncombustible which includes a battery, a heating element, and a liquid compartment, usually containing addictive nicotine, that is added to the e-liquid or included in the device. The heating element aerosolizes the liquid for the inhalation of the liquid nicotine or other contents.
In the vaping liquids that contain nicotine, the level of nicotine can vary widely. Prior to July 2021 in Canada, vaping substance nicotine strengths ranged from 0 to over 60 mg/ml of nicotine. Since July 2021, the Nicotine Concentration in Vaping Products Regulations establish a maximum concentration of 20 mg/mL for vaping products manufactured in or imported into Canada. Market size is clearly affected by national laws governing the sale of e-cigarette products. In 2016, the Washington State Legislature passed a law (RCW 70.345) to regulate lipid vapor of electronic cigarettes products.
Vaping has exploded into a national crisis, and tobacco companies are helping to fuel it by targeting kids with flavors such as gummy bear and cotton candy. E-cigarette companies have promoted unsubstantiated health claims about their products as healthier than traditional cigarettes, when, in fact, e-cigarettes are uniquely dangerous for kids due to nicotine’s impact on their developing brains. Since being introduced to the U.S. market in 2007, e-cigarette use among youth has increased to epidemic levels (Surgeon General's Advisory on E-cigarette Use Among Youth, 2018). The availability of flavored e-cigarettes is one of the most commonly cited reasons for e-cigarette use among youth (Tsai et al., 2018). Given the importance of flavors contributing to use of e-cigarettes among youth, these data briefs build on this previously published article, providing an update on trends in unit sales of e-cigarettes in the U.S. by product and flavor type.
A 2021 review found people who used e-cigarettes to quit smoking, as well as having expert face-to-face support, can be up to twice as likely to succeed as people who used other nicotine replacement products, such as patches or gum. Vaping involves using a device known as an e-cigarette—also called a vape pen, mod, or tank—to heat up a small amount of liquid, turning it into a vapor that can be inhaled. Most vape liquids contain substances such as propylene glycol and glycerol as base ingredients that create the vapor.

E-cigarettes are known by many different names, and sometimes people find it hard to understand what is really known about these devices.
But if you find using an e-cigarette helpful for quitting and staying smoke-free, it's much safer for you and your baby than continuing to smoke. It's also important to choose the right strength of nicotine in your e-liquid, which will depend on how much you smoke. You can get advice on models and e-liquids from a specialist vape shop. You will not get the full benefit from vaping unless you stop smoking cigarettes completely. E-cigarettes do not burn tobacco and do not produce tar or carbon monoxide, two of the most damaging elements in tobacco smoke. At Yale Medicine, most pediatricians have started asking middle- and high school-age patients about their exposure to vaping, in large part to identify patients who might be at higher risk for developing related problems.

If you're pregnant and need support to quit smoking, licensed nicotine replacement therapy products like patches and gum are the recommended option. Making small changes to your vape products or how you vape should help. Side effects are usually easily managed and should not stop you from vaping as a way to quit smoking. You're roughly twice as likely to quit smoking if you use a nicotine vape compared with other nicotine replacement products, like patches or gum.
Short-acting nicotine replacement therapies — such as nicotine gum, lozenges, nasal sprays or inhalers — can help you overcome intense cravings. These short-acting therapies are usually safe to use along with long-acting nicotine patches or one of the non-nicotine stop-smoking drugs. Mothers Against Vaping told Mint that they fear for their teenage children.
However, the long-term effects of all flavour chemicals used by this industry (which are more than 15,000) remain unknown and they are not usually included in the product label [78]. Furthermore, there is no safety guarantee since they may harbour potential toxic or irritating properties [5]. In addition to its toxicological effects on foetus development, nicotine can disrupt brain development in adolescents and young adults [44,45,46]. Several studies have also suggested that nicotine is potentially carcinogenic (reviewed in [41]), but more work is needed to prove its carcinogenicity independently of the combustion products of tobacco [47].

Juul’s C.E.O., Kevin Burns, who is fifty-four, has a friendly dad-who-loves-his-vacation-house demeanor. Burns described Juul to me as a "cigarette-killing company." Before he accepted the job, he said, he convened an informal focus group in his kitchen with his son, who’s in high school, and a few of his son’s friends. He asked them why they had these things, when they got them, how prevalent they were. "We have frustrations about how the product is glorified on social media," he told me. I admitted to Winickoff that I was probably endangering my lungs as a weed smoker.
