More research needs to be done to understand the long-term health effects of secondhand exposure to e-cigarette vapor. Secondhand exposure to e-cigarette vapor is said to be less toxic than secondhand exposure to cigarette smoke. However, secondhand vapor is still a form of air pollution that probably poses health risks. Youth vaping has declined from all-time highs in recent years, with about 10% of high schoolers reporting e-cigarette use last year. Vaping is not completely risk-free, but it poses a small fraction of the risk of smoking cigarettes. The aerosols generated by ENDS typically raise the concentration of particulate matter in indoor environments and contain nicotine and other potentially toxic substances. ENDS emissions therefore pose potential risks to both users and non-users. Accidental exposure of children to ENDS e-liquids pose serious risks as devices may leak or children may swallow the poisonous e-liquid.

The FDA lists 93 harmful or potentially harmful 42 chemicals identified in electronic cigarettes found in regular cigarettes, and the National Cancer Institute (NCI) describes cigarettes as having more than 7,000 chemicals in them. Because e-cigarettes contain fewer chemicals the industry has presented them as a healthier alternative to regular cigarettes. But vape liquids can still contain nicotine, a highly addictive drug.

Use of microporous ceramic as a wicking material improves heating efficiency, but how it affects the chemical emissions of these devices is unclear. We assessed the emissions of a pod e-cigarette with innovative ceramic wick-based technology and two flavoured e-liquids containing nicotine lactate and nicotine benzoate (57 and 18 mg mL−1 nicotine, respectively). Among the studied harmful and potentially harmful constituents (HPHCs) listed by the US FDA and/or WHO TobReg, only 5 (acetone, acetaldehyde, formaldehyde, naphthalene and nornicotine) were quantified at levels of 0.14 to 100 ng puff−1. In the combustible cigarette (Kentucky reference 1R6F), levels were from 0.131 to 168 µg puff−1.
All this evidence would suggest that e-cigarettes are potentially less harmful than conventional cigarettes (Fig. 2) [11, 14, 22, 24, 27,28,29]. Importantly, however, most of these studies have investigated only short-term effects [10, 14, 15, 22, 27,28,29, 31, 32], and the long-term effects of e-cigarette consumption on human health are still unclear and require further study. Tetrahydrocannabinol (THC)-containing e-cigarette or vaping products was linked to most EVALI cases. Emergency department visits related to e-cigarette, or vaping, products have sharply declined in recent years, but the CDC continues to monitor reports. In 2022, e-cigarettes remained the most common tobacco product used by high school (16.5%) and middle school (4.5%) students in the last 30 days. And more than a quarter (27.6%) of current youth e-cigarette users say they use an e-cigarette product every day.
The U.S. Centers for Disease Control and Prevention recommends a combination of counseling and medications as the best strategy for quitting smoking. Vaping companies have long claimed their products can help blunt the toll of smoking, which is blamed for 480,000 U.S. deaths annually due to cancer, lung disease and heart disease. The FDA has not authorized any flavored e-cigarette products and reports that it has denied marketing applications for millions of flavored products. E-cigarette companies have filed over 60 lawsuits challenging the FDA’s marketing denial orders. In addition, while the FDA denied marketing applications for all Juul products in June 2022, it subsequently put that decision on hold in the face of a lawsuit by Juul. When e-cigarette products first became popular, we didn’t have much information about health risks.
Replacing your cigarette with a cigar, pipe, e-cigarette, or hookah won’t help you avoid the health risks. The U.S. government has taken steps to restrict most flavored vaping products, effective in 2020. While this should reduce the number of flavored cartridges available in the United States, the guidelines are limited to products submitted for market authorization and don’t include those made at home or purchased online from other countries. Other studies suggest that while using e-cigarettes sends pollutants into the air, the concentration of these toxins drops quickly after someone stops vaping (much faster than cigarette smoke), but that doesn’t mean that the risk has gone away. Some, for example, will settle on surfaces, such as carpets or furniture, where small children might touch them or inadvertently ingest them when placing contaminated objects in their mouths.

Among girls, only a higher proportion of never-users in the school remained protective for S-SM and S-EC. Among boys, the same was observed for S-EC, whereas a higher proportion of students with positive attitudes towards snus use in one’s age group increased S-SN. Interestingly, a higher proportion of students planning for general upper secondary education had lowered S-EC and S-SN in the univariate analyses but increased S-SM and S-SN in the multivariate models. This may reflect, for instance, more complex interactions between individual- and school-level factors influencing susceptibility among boys. In general, students with positive attitude towards product use in one’s age group and current use of some other tobacco or nicotine product had consistently higher susceptibility regardless of product type.

In our study, there were between-school differences, which appeared larger for girls than boys. In the univariate analyses, most school-level factors either increased or lowered susceptibility.
